This form grants authority to treat in the event of a medical emergency. Please print, complete, scan or take a picture of the completed form, and upload.

Code of Conduct

Expectations for all rowers. Please make sure you are signed in as the athlete (not another family member), read the code of conduct, and then ‘sign it’ by typing your name.

Participation Agreement

Required on file prior to participating in any OAR-related activity.

Safety Test: Float + Lifejacket

OAR Float Test form to be filled out by Lifeguard conducting the exam. Detailed instructions here.

Sports Physical (club code LMACX). Update takes 1-2 days.

Annual sport physical is required. Please upload either this completed form or the form your doctor used for the completion of your physical.

US Rowing Membership

'Learn-to-row" level is required for classes. "Basic" is required for annual participation at OAR. Club code is LMACX. If the rower is looking to compete, they'll need to upgrade the level. It may take a day or two for the membership status to update in ICrew.

USRowing Waiver (club code LMACX)

Renewed annually through USRowing directly for as long as the rower is rowing. Club code is LMACX
